Reports - Quality Indicators Tab

The Quality Indicators reports (Antipsychotics and Polypharmacy) have been developed in line with the quarterly mandatory clinical audits and reporting requirements - where nursing staff are required to collect data.

StrongCare has replicated the reporting parameters from the Aged Care Quality and Safety Commission to streamline this data collection.


Antipsychotics Report

The first report is the Antipsychotics report.

  • Select the Antipsychotics report by clicking on the button. Take a moment to read the instructions

Further instructions are provided. This is a digital replication of the Antipsychotic Quality Indicators report from the Aged Care Quality & Safety Commission. 

  • Enter a date into the field then apply the required one week date range

The collection date for the quarter that you select populates into the reporting requirements field. The Quality Indicator reporting tool auto calculates the number of care recipients assessed for antipsychotic medications during this timeframe, as it pulls this data from the StrongCare database. On the right, the report pulls the list of the care recipients.

  • You will need to select Yes or No to the column titled Care Recipient Admitted to Hospital? When Yes is selected, the Care Recipients Excluded field will be updated
  • Continue to select the resident’s hospital criteria

The next column has been updated based on the data completed  from the nursing Administration fields for the resident. If an Antipsychotic was received by a resident this column will auto populate with the correct data, either Yes or No.

You may manually update these fields if required. The data in this column will populate numerically into the reporting field.

You will need to determine if the resident received an Antipsychotic for Medically Diagnosed Psychosis. 

  • Use the dropdown arrow to select Yes if this has been determined. The results will populate into the reporting field
  • You are then ready to click the Submit the Report button
  • Then click Close

Polypharmacy


Under the Quality Indicators tab, 

  • Click on the Polypharmacy button

The StrongCare Polypharmacy Quality Indicators report incorporates the mandated exclusion criteria for the medications indicated – it excludes Short Term Meds, Creams, Vitamins, PRNs and Supplements, etc.

  • Then select a date by clicking into the date field and activating the calendar
  • The report parameters then load. Take a moment to read the Instructions
  • The date you selected populates into the reporting requirements field
  • To the right of the screen the StrongCare report will pull a list of active residents for that date

The number of residents will auto populate into the Total number of care recipients assessed for polypharmacy on that date.

  • Use the dropdown arrow to indicate if the resident was admitted to hospital on this day
  • Ensure you select Yes or No for each resident listed

If Yes was selected, the ‘number of care recipients excluded’ field will be updated.

The report will calculate the Polypharmacy requirements and incorporate the exclusion criteria  and this field below will be updated. 

  • Then click the Submit Report button 
  • Click Close

To confirm Polypharmacy reporting you can return to the residents Medication Chart. Simply check the date and Chart Type to ensure that either Non Prescription Meds or Nutritional Supplements were not included in the Medication Chart for the resident as these items will be excluded from the Polypharmacy report.

Again review the saved reports under the Previous Reports tab.
